Legendz AMOE: blue-ink request cards

Prepare Legendz's generated statement, 11-digit code and dated 4 by 6 inch request.

An eligible Legendz account; blank, white, unlined and unfolded 4 by 6 inch paper; stamped #10 envelope.
Use blue ink throughout. Put only one request card in each envelope.

Prepare the request

Get your code and statement
Sign in and open the Postal Request Code option, then follow the prompts. Save the unique 11-digit code and its generated statement. The code expires 45 days after display and must still be valid when the request is processed.
Write one side
Date at the top right, code at the top left. Below the code, write your legal name matching your ID and account, account email and registered residential address, then copy the generated statement with its code exactly.
Prepare the envelope
Handwrite your return address and Sweeps Coin Request Card on the front. Insert one card. Use the destination below and mail from the same state or province as your verified residential address.
