MyPrize.us AMOE: handwritten mail-in requests

MyPrize’s postal request needs a signed, dated card sent within three days of signing.

MyPrize US’s official help rules allow one mail-in request per calendar-month sweepstakes period, for 4 Sweeps Cash if accepted. The signature and mailing dates both matter.

Materials

  • Verified MyPrize US account.
  • White, unlined, unfolded 4 × 6 inch index card; white, unlined, unfolded #10 envelope; pen and postage.

Prepare and send

  1. Write the card

    On one side, handwrite legal name, account email, username and residential address, in order. Copy MyPrize US’s current declaration and insert the current contest month and year.

  2. Sign and date

    Immediately beneath the declaration, sign the card and date it in MM/DD/YYYY format. The signature date must fall within the current sweepstakes month.

  3. Mail promptly

    Hand-address the envelope to:

    Enclose one card. Mail from your verified state within three days of signing.
